It enables rapid identification of at-risk patients and access to their medical history, thereby enabling rapid diagnosis and treatment especially in emergency situations. Classic examples are people with diabetes and/or heart problems who have high risk of collapsing and having attacks. VeriChip is also useful in vehicular accidents and other trauma incidents where the victims aren't capable of answering questions. In cases of large-scale catastrophes, VeriChip facilitates tracking and identification of victims. According to a coroner in Mississippi, VeriChip helped identify victims during the Hurricane Katrina incident.</p><p>Earlier this year, Google Health was launched, an online personalized health records service. Google Health is based on the principle that since it's the patient's medical record, the patient should control it, decide what should be in it, and who gets access to it. One of the features of the service includes records from hospitals and pharmacies that are Google Health-enabled or are registered Google Health partners.</p><p>The HealthVault is another online health information storage service offered by Microsoft with features similar to Google Health. Here is what they have to say:</p><p>(1) EMRs threaten our privacy. In this day and age when people's mantra is "I need my privacy", not many people are comfortable about having their entire medical history recorded and digitized for almost just anybody to see - in other words, incursion into people's privacy. The confidentiality of doctor - patient relationship is still sacrosanct. Besides, medical data can be used against a person in some cases - be it for a job application, insurance coverage or a college scholarship. Although it is against the law to discriminate against people with illnesses and disabilities, it is a fact of life that the fitter you are, the more competitive you are in the job market. The planned incorporation of genetic data in EMRs further adds to people's fear of incursion into their private sphere.</p><p>(2) EMRs can lead to loss of the human touch in health care. In the process of digitalization, the interpersonal aspect in health care may be lost. In handwritten hospital charts, doctors and other health care practitioners may write what they think and they feel based on their personal observations in their very own words. EMR is simply about ticking off boxes and crossing out things in electronic forms. The doctors are forced to think in categories and can seldom express a personal opinion on an individual case. Because of the lack of flexibility of many electronic reporting systems, cases of misclassification of patients and their conditions have been reported.</p><p>(3) EMRs are not that efficient. Despite efforts in digitalization and standardization, EMRs are actually far from being standardized and not as efficient as it is purported to be. It often happens that one clinic's EMR system is not compatible with that of a general practitioner or another clinic's system, thus belying the claim of added efficiency. In addition, not all users of EMRs are satisfied with the current state of the art. Although the objective is mainly efficiency and healthcare quality, one study showed that nurses in the Netherlands are not completely satisfied with their EMR implemented in 2006-2007.</p><p>(4) EMRs are not safe and secure. Google Health and HealthVault are quick in assuring patients of the safety of their online health accounts. Access to the patient's account is only possible using log ins and password. In addition, HealthVault assures that "all health information transmitted between HealthVault servers and program providers' systems is encrypted" and that Microsoft does it best to use the "highest standards of security to safeguard consumer health information from theft, loss, or damage."</p><p>However, there are cases wherein passwords and encryptions do not seem to be adequate as data protection tools. Stories of data hacking, stolen identities and blackmail abound. Even high security databases such as those run by banks and credit institutions are often compromised. This impression was aggravated by the many well-publicized incidences of data loss or breach. A few examples are listed below:</p><p>November 26, 2007, Canada. Hackers accessed medical information on HIV and hepatitis from a Canadian health agency computer. - September 22, 2008, UK. The National Health Service (NHS) reported the loss of 4 CDs in the mail containing information on 17,990 employees. - September 30, 2008, US. The company Blue Cross and Blue Shield of Louisiana confirmed breach of personal data, including Social Security numbers, phone numbers and addresses of about 1,700 brokers. The data was accidentally attached to a general email.</p><p>In addition, there is criticism over Google Health not being a "covered entity under the Health Insurance Portability and Accountability Act of 1996 and the regulations promulgated thereunder (HIPAA)" under its terms and conditions and is therefore not subject to HIPAA privacy of individually identifiable health information. The HealthVault terms and conditions do not mention HIPAA privacy laws so it is not clear what its status is regarding this issue.</p><p>(5) VeriChip is not for humans. It is to be expected that although many of us are amenable to the use of RFID chips in pets, the idea of implanting similar chips in human beings is bound to raise hackles in humans, no matter what the US FDA says. A big opponent of the VeriChip and similar chips of its kind is the consumer advocacy group Spychip.com. In a position paper, Spychip and many advocacy and consumer awareness groups see RFID tagging (be it on your person or on the items you buy) as a major threat to privacy and civil liberties. They see the tagging as some kind of "Big Brother" operation. Without doubt, we have the technology to make EMRs standardized and efficient. Immediately following this leg wobbling set of squats, do as many squat-leaps up to 15 as possible.</p><p>Hamstring curls or Stiff-legged deadlifts 1 x 50</p><p>Lateral Box Leaps 1 x 20</p><p>Stair run 2 x 30 steps sprint</p><p>Instructions: If doing hamstring curls, use enough weight to keep a light resistance on the legs. If stiff-legged, use just a light pre-loaded bar of 15-20 pounds for 50 reps. Keep tight form throughout sets of either of these.</p><p>Immediately run stairs up 30 steps, and down (many gyms have stairs) - if not use an aerobic step apparatus and go up down up down up down up down, alternating feet as quickly as possible. Then immediately go to box leaps. Make sure your first try is using an 8-12 inch step that is about as wide as it is tall. With both feet together, leap from side to side to side to side for 20 reps.</p><p>You may think to yourself... I'm not sure this will increase my mass. Well, just wait! It not only can increase mass, but it can reshape the mass you have and make you more functional, explosive and dynamic in the gym. This notion that wealth and well-being are related is also supported by a microeconomics theory that states that an increase in the income level of a society would lead, other things being constant, to greater well being (Easterlin & Christine, 1999).</p><p>However, it is important to keep in mind that increases in individual income levels are relative to the changes in one's reference group (Lian & Fairchild, 1979). Increases in income are considered to be relative. In other words, if an individual's gains in economic status outpace the gains of the reference group then the individual will likely experience a greater sense of satisfaction. On the other hand, if their gains are equal to the average in their reference group, there will likely be no change. If the increases are less than the reference group than the result will be less satisfaction. Therefore, it may be important for many older adults transitioning to retirement to have adequate savings or other income in order to maintain or exceed their previous financial status.</p><p>The relationship between education and income to successful aging is a complex one that involves numerous external variables. But it seems that there is conclusive evidence that both education and income levels help to prepare an individual for the changes that they will face in old age and "influence on their ability to view aging as an opportunity for continued growth as opposed to an experience of social loss" (Steveink, Westerhof, Bode, et al, 2001).</p><p>One of the most important aspects of how well individuals age is related to their ability to develop and maintain strong relationships and social support systems (Rowe & Kahn, 1998). It is also important to mention that solitude, or a lack of social interaction, is considered a major health risk factor (Unger, McAvay, Bruce, et al, 1999). Recent studies suggest that the effects of social ties on the risk of physical decline in elderly are greater in men than women. These studies also report that there is a strong relationship between social support or social networks to the probability to cardiovascular and all cause mortality for men (Berkman, Seeman, Albert, et al,1993).</p><p>This gender difference could be explained by the fact that women devote a greater portion of their lives caretaking and developing friendships, so they are more accustomed to building and utilizing social networks. While men, in contrast, have devoted a greater portion of their lives to their careers, therefore, they have not developed the social networks or skills to utilize these networks that most women have (Unger, McAvay, Bruce, et al, 1999). In addition, social ties appear to be most important among elderly individuals with less physical ability (Unger, McAvay, Bruce, et al, 1999). It seems that people with physical disabilities have a greater need to develop friendships and support networks to assist them in coping with the limitations caused by their conditions. Friends and family provide them with a means to continue participating in social activities and complete the tasks of everyday living that they may be unable to accomplish on their own. This provides support for the belief that establishing strong social networks may increase not only quality of life, but quantity as well.</p><p>Social relationships and social support systems serve as protective factors in many ways (Bovbjerg & McCann, et al, 1995), (Krause & Borawski-Clarke, 1994). They benefit individuals by enhancing self esteem, providing encouragement, and promoting healthy behaviors. It is also possible that social networks may provide more tangible assistance such as food, clothing, and transportation. This type of assistance enables an elderly person to remain socially active even though they may not have the means to do so on their own. It is also important to distinguish the difference between receiving support and assistance from friends or relatives as opposed to agency assistance.</p><p>Possibly the most important source of social support comes from the family, which provides self-system mechanisms which increase an individual's subjective impression of life satisfaction. In addition families provide a system of support and interaction that may not be available from outside sources for some elderly people. All of these types of networks may prevent the degree of social isolation in old age, that is associated with depression and other psychological problems (Krause, 1991).</p><p>With all of the physical and psychological changes that people face in late adulthood i.e., decreases in vision, hearing, memory, etc., the ability to adapt to life circumstances that force aging individuals to move from one living style to another is an integral part of successful aging (Warnick, 1995). Simply maintaining the ability to perform the everyday tasks of living is not necessarily considered successful aging. Successful aging requires the maintenance of competence involving cognitive, personality, material, and social resources (Baltes & Lang, 1993). Adapting to these changes requires the use of flexible strategies to optimize personal functioning (Baltes & Baltes, 1990).</p><p>The strategies that one may employ to cope with the changes that accompany the aging process may be limited not only by the individuals ability to utilize a new strategy, such as learning sign language or walking with a cane, but also by their perception of their ability to do so. Many elderly people will avoid using new tools to adapt to change if they believe that they are unprepared to make such an adjustment (Slagen-DeKort, 2001).
<br>Perceived self efficacy is defined as "peoples judgment of their capabilities to organize and execute the courses of action required to attain designated types of performance" ( Bandura, 1986). People who believe in their ability will set higher goals for themselves and expect that they will be able to achieve these goals. Self efficacy has been found to influence the adaptive strategies used by older adults (Slangen-DeKort, 1999).</p><p>There are two dispositions besides perception of self efficacy that influence individuals ability to cope, these are flexibility and tenacity (Slangen-DeKort, 1999). Tenacity is defined by an individuals persistence with which they are able to remain focused upon their goals in the face of obstacles. Flexibility refers to ones ability to readjust goals based on new information. The research of Slangen-DeKort et al (1999) concludes that self referent beliefs regarding personal competence influence adaptive behavior and the choice of adaptive strategies. "The direct effect, which is strongest, implies that even if a person appraises a certain adaptation as the most optimal one, this adaptation may not be adopted when this person perceives that the required efforts exceed his or her personal competence. In this case, a less optimal alternative strategy will be embraced." (Maddox & Douglas, 1973).</p><p>Given the enormous number of variables that are involved in determining how well an individual will age, it is impossible to point to one factor as being the most important. But, it is safe to say that ones ability to successfully age is determined to a great extent by their attitudes toward aging and growing old. These positive and negative attitudes will be the result of how effectively an individual is able to adapt to the physical, psychological, and social changes that will take place throughout adulthood. If someone is able to accept the changes of life and look forward to the challenges that they present with hope and desire to change, then they will be better prepared to face old age. In addition, the relationships and beliefs that are developed across the life span will be relied upon in old age as a resource for support and assistance in coping. Upon examining research on successful aging, it seems that many of the concepts that are applied to earlier developmental stages are equally important in old age.</p><p>For example, change, adaptation, personal growth, and cognitive function are aspects of development that may be as important in old age as they are in childhood development. According to Bennis, Spreitzer and Cummings (2001) in the future the landscape of health care organization will become more decentralized, which will promote agility, proactivity, and autonomy. Future leaders may move away from singular roles to shared leadership networks that may themselves alter the foundations of the organization. The demands for shared leadership or leaders shifting roles on teams will continue to increase. Health care organization will foster the development and empowerment of people, building teamwork and shared leadership on all levels. The leaders of the future will be guides, asking for input and sharing information. Telling people what to do and how to do it will become a thing of the past (Bennis, Spreitzer and Cummings, 2001). In the 21st century the dynamics of health care will offer leaders who have the ability to motivate and empower others a platform to maximize an organization human resources. Leadership will have to be committed to encourage a two way communication in which the vision meets both the organizations objectives and the employee's needs. Kerfoot and Wantz (2003) suggested in inspired organizations where people are committed and excited about their work, compliance to standards and the continual search for excellence happens automatically. In these organizations, compliance continues when the leader is not present. This type of leadership requires the team leader to use all available means to create three conditions among individuals: (a) shared purpose, (b) self-direction, and (c) quality work. Leaders who create commitment among their employees believe in creating a shared vision that generates a sense of shared destiny for everyone (Kerfoot & Wantz, 2003).</p>

Sharing information promotes a sense of participation and allows people to feel acknowledged and respected (Atchison & Bujak, 2001 p. 141).</p><p>Toseland, Palmer-Ganeles, and Chapman (1986) suggest when individual leaders cooperate and share their expertise and skills, a more comprehensive decision making process can be achieved rather than when leaders work independently. For example, in a geriatric team, a psychiatric nurse may lead a group focused on heath concerns, a social worker may lead a therapy group, or a mental-health therapy aide may lead a structured reality-orientation group (Toseland et al., 1986). Shared commitment form the leadership in the future will help to develop, coordinate, and integrate the complex and ever changing health care setting for the 21st century.</p><p>Respect for authority and work ethic</p><p>Haase-Herrick (2005) suggested shared leadership gives the opportunity to enhance or build trust among individuals. Leadership is mobilized around refining the roles of individuals creating positive health practice environments that support the work of the group (Haase-Herrick, 2005). Leadership ability to lead a team in ways that build morale and reinforce work ethics empowers others to perform to their potential in a group. Leadership is the ability to lead individuals towards achieving a common goal. Leadership builds teams and gains the members shared commitment to the team process by creating shared emotion within the group (Pescosolido, 2002).</p><p>Collaboration among leaders in health care</p><p>There are new models that are emerging which add a new perspective on how to produce effective collaboration within leadership. Wieland et al., (1996) discussed transdisciplinary teams in health care settings, where members have developed sufficient trust and mutual confidence to engage in teaching and learning across all levels of leadership. The collaborating is shared but the ultimate responsibility for effectiveness is provided in their place by other team members. The shared responsibility for example might be a situation where clinicians on a team each serve in a leadership role regardless of their particular disciplinary expertise (Wieland et al., 1996). The shared commitment model of leadership allows for the independence and equality of the contributing professions while pressuring team members to achieve consensus about group goals and priorities. It is important to emphasize the importance of collaboration in a complex and changing health care environment. The focus on the primary purpose for partnership of leaders will ultimately rest on the shared belief in meeting organizational goals though a collaborative effort. Atchison and Bujak (2001) suggest it is important to reemphasize the importance of keeping everyone informed on the primary purpose of achieving success though a collaborative effort. Clarifying expectations and specifically illustrating how proposed changes are likely to affect the participants is important in achieving commitment leadership (Atchison & Bujak, 2001)</p><p>Leadership competency on all levels</p><p>The ability to lead in the 21st century requires leaders to be competent in motivating and empowering others to perform to their maximum potential. According to Elsevier (2004) leadership is the ability to lead a team or number of individuals in ways which build morale, generate ownership and harness energies and talents towards achieving a common goal. The leadership competency is all about motivating and empowering others while accomplishing organizational objectives. Leadership is the vehicle in which the vision is clarified though the encouragement of two-way communication on all levels of the organization (Elsevier, 2004).</p><p>Leaders in the 21st century will have to be competent in identifying change as they occur and encourage others to adjust to those changes for the mutual benefit of achieving objectives. Elsevier (2004) suggest leaders will have to be comfortable with change because which change comes new opportunities for collaboration among followers and peers (Elsevier, 2004). Improving the results of change initiatives while making sure those changes are fully understood will be a priority for leaders who choice to lead by commitment leadership.</p><p>Leadership as a changing agent</p><p>Longest, Rakich and Darr (2000) suggest organizational change in health care organization does not occur absent certain conditions. Key are the people who are catalysts for change and who can manage the organizational change process. Such people are called change agents. Anyone can be a change agent, although this role usually is played by leadership. Change agents must recognize that any organizational change involves changing individuals. Individuals will not change with out motivation introduces by the changing agent. The changing agent must create a body of shared values and attitudes, a new consensus in which key individuals with in an organization reinforce one another in selling the new way and in defending it against opposition (Longest, Rakich and Darr, 2000). As health care organizations change in the 21st century successful leaders must have the skills that are necessary to make change possible with in teams of individuals. Longest, Rakich and Darr (2000) suggest one of the important category of change is team building or team development, which "remove barriers to group effectiveness, develop self sufficiency in managing group process, and facilitate the change process (Longest, Rakich and Darr, 2000). A leader who leads by commitment must seek to minimize the resistances to change by building a consensus of objectives with in the organizations culture.</p><p>Conclusion</p><p>Leadership in the complex health care environment in the 21st century will need individuals to be committed to the promotion of team effectiveness. Sarner (2006) suggest leadership is a "power- and value-laden relationship between leaders and followers who intend real changes that reflect their mutual purposes and goals." In plainer language, leadership is the dynamic that galvanizes individuals into groups to make things different or to make things better -- for themselves, for their enterprise, for the world around them. The essential components of leadership have remained more or less constant: intelligence, insight, instinct, vision, communication, discipline, courage, constancy (Sarner, 2006). In the 21st century leaders must know how to gather, sort, and structure information, and then connect it in new ways to create clear objectives that satisfy both the organization and individuals needs. The important skill that can be learning during this process of leadership is the ability to listen to colleagues and collaborators for the sole purpose of foster a shared consensus. These are two opposite but complementary forces and, together with qi (vital energy), they control the universe and explain the relationship between people and their surroundings. Imbalance in these two forces, or in the qi, results in illness.</p></strong>

Astin (1998) also agreed that it was seen as being more compatible with the patients' values, spiritual and religious philosophy, or beliefs regarding the nature and meaning of health and illness.</p><p>In traditional Chinese culture, taking medication is thought to be aversive, hence medications tend to be taken only until symptoms are relieved and then discontinued; if symptoms are not obvious, medications will probably never be taken.</p><p>Apart from parental cultural beliefs, minor side effects of certain antibiotics such as stomach upset may contribute to the poor adherence of medication. The use of "leftover", "shared" antibiotics and over-the-counter purchase of antibiotics by parents are common situations in the community.</p><p>They think that their children suffer from the same illnesses judging by the similar symptoms, so they would give the "leftover" or "shared" antibiotics to their children and only bring them to their doctors if there is no improvement (Chang & Tang, 2006). This may cause their conditions to deteriorate and may necessitate aggressive treatments later which may have unnecessary side effects.</p><p>However, there are small groups of Chinese who also blamed ill-health or misfortunes on supernatural forces, or on divine retribution, or on the malevolence of a 'witch' or 'sorcerer' (Helman, 1994). Such groups will usually seek cures from their religions.</p><p>In Singapore, the Ministry of Health has drawn up the TCM Practitioners' Ethical Code and Ethical Guidelines to prevent any unscrupulous practitioners from preying on their patients and taking advantage of their beliefs, for example, molesting ignorant patients.</p><p>The degree of acculturation has been evidenced in the following case. An old man was brought into our hospital with a week-long history of malaise, nausea and vomiting, and sudden jaundice. He was diagnosed to have an obstructive mass in the liver.</p><p>A biopsy revealed hepatocellular carcinoma. The serological test suggested chronic active hepatitis B. When the news broke to his son that his father had cancer, he requested not to disclose that to his father.</p><p>When we discussed end of life issues such as hospice care and "do-not-resuscitate" (DNR) orders, the son tried to divert the discussion to other issues such as when his father could go home.</p><p>Cultural Issues that may be involved in this case are:</p><p>The Chinese tend to protect the elderly from bad news.</p><p>Believing in karma - the older folk believe that discussing illnesses or death/dying is bad luck. They have an opportunity to design a system that can shine as a jewel in the crown of universal health care systems everywhere. </p><p>However, all ethical decision making is structured around values. In order for universal health care to be embraced by all citizens in the United States, they will first have to agree to the collective value of equity and fairness and embrace the goal of meeting their collective responsibility to each other while maintaining individual rights and freedoms. That may prove to be the most difficult obstacle of all.</p>
